Accidents/Incidents Investigation
All accidents are preventable.
An accident is an unplanned, undesired event leading to injury or damage.
It is essential to carry out thorough investigation of all incidents and accidents, no matter how trivial, for one simple reason - to prevent re-occurrence.
We can assist your Company with incident investigation and identification of route cause(s). The level of detail required and the route cause analysis method used depends on the seriousness and complexity of the incident. There are two main methods that we use to identify the route cause of an incident:

Fault Tree Analysis starts with defining what the `Top Event` is. Proper definition of the `Top Event` is key to carrying out a good Fault Tree Analysis. Once the top event is defined, the next step is to ask `Why?` the top event occurred. This will generally provide the immediate cause and to get to the route cause, the question `Why?` is asked repeatedly against each identified immediate cause.
- Events and Causal Factors Analysis method is used fore more complex incidents. Two basic elements are considered in this method: Events (occurrences or happenings within a time and date) and Conditions (factors that influenced and contributed to the event).
Once the route causes have been identified and classified, corrective and preventative actions should be assigned against each route cause.
We will also assist you with completion of IR1 or IR3 forms for submission to the Health and Safety Authority, if required.
